Please fill out intake forms and bring with you to your first chiropractic visit at Symmetry Health. If you are unable to do so, please arrive 15 minutes prior to your first appointment to fill out the necessary paperwork.
Naturopathic Medicine New Patient Packets:

New patients to one of our Naturopathic Doctors are asked to please download, print, and complete the appropriate intake packet. Please mail or fax your completed packet to Symmetry Health (858) 270 - 6898 at least one week prior to your visit to allow your doctor to appropriately prepare for your visit. If you do not wish to download the forms, please stop by the office to pick up a packet to be completed 1 week prior. If necessary paperwork is not received 1 week prior to your scheduled initial visit, we may contact you and reschedule your initial visit. If your initial visit requires immediate attention, please inform our front desk and we can make an exception. Thank you for your cooperation!

What to expect of your first chiropractic visit at Symmetry Health:
Your first chiropractic visit at Symmetry Health will take between 30 minutes and one hour depending upon your individual presentation. New patients can expect to answer questions pertaining to their current and past illnesses, and their family, social, lifestyle, and dietary history. From this information, Dr. Mick can assess risk factors, determine which physical and orthopedic exams to perform, and if imaging or blood work is warranted at the first visit. After the patient's history is assessed and physical exam is performed, Dr. Mick will establish if the patient is a good candidate for treatment upon the first visit. Treatment modalities used during the first visit often include physiotherapy, soft tissue manipulation, physical rehabilitation, and a chiropractic adjustment, although treatments are subject to the doctor’s clinical judgment. Follow-up appointments are recommended so that Dr. Mick can see how patients respond to initial treatment and will allow the doctor to effectively keep track of each patients progress.

What to expect of return chiropractic visits with Dr. Mick Wilson:
Follow-up visits generally take much less time and may last anywhere from 10 - 30 minutes depending on the complexity of the case, if physical exams need to be repeated, and which forms of treatment are being delivered. Additionally, long term care with Dr. Wilson can provide:
- Physical rehabilitation
- Exceptional patient education and home treatment recommendations.
- Improved mobility, strength, and flexibility.
- An improvement in overall quality of life.

What to expect of your first naturopathic visit:
Your first visit with Dr. Danielle Schwaderer will last approximately 90 minutes. This is the amount of time necessary to obtain a comprehensive history, physical exam, appropriate diagnosis, order laboratory work and imaging if necessary, create an individualized treatment plan, and answer all patient questions. Throughout the first visit, patients will be asked questions regarding their personal health, family, social, dietary, and lifestyle history. An accurate patient history is needed in order for the doctor to make an appropriate assessment of risk factors, obstacles to cure, root causes of illness, and overall health of the patient. At the end of the visit, an individualized treatment plan will be suggested to the patient, which may include a botanical formula that was created specifically for the patient, a homeopathic remedy, bio-identical hormone prescription, compounded flower essence therapy, and patient education & resources. Dr. Danielle Schwaderer has a wide variety of botanical medicines, homeopathic remedies, supplements, and flower essences that are available for their patients to purchase at the time of visit.
If necessary, a referral to another health care provider will be provided to ensure the most comprehensive care possible!

What to expect of return naturopathic visits at Symmetry Health:
- Visits lasting 30 - 45 minutes.

Developing a therapeutic relationship with your doctor.

A review of your current health status and progress from the time of last visit.

Interpretation of lab results or ordering any new laboratory work that may be indicated.

Physical exams repeated if necessary or new exams performed depending on symptom presentation.

Each return visit gives Dr. Danielle Schwaderer an opportunity to better understand how health and illness manifest within you. Follow up visits allow your doctor to see how you have been responding to treatment, what your triggers and obstacles to cure may be, and ultimately how to effectively balance and prevent future disharmony and illness.
Naturopathic Medicine Financial Policies at Symmetry Health:
At this time, Naturopathic Doctors are unable to be contracted providers on insurance panels in California, thus all fees are due at the time of service. Dr. Danielle Schwaderer will create a super bill with appropriate medical codes for each visit, in which patients may submit to their insurance provider for reimbursement. We suggest that patients interested in receiving reimbursement for naturopathic medical services call their insurance provider to inquire about reimbursement for out of network providers.
